In short

Clinical operations covers the running of a trial — site selection and management, monitoring, data management, vendor oversight and the operational documentation that evidences all of it. The regulatory expectation is GCP under ICH E6, and the recurring pressure is demonstrating consistent oversight across sites and vendors rather than at any single one.

Study Start-Up: The Period That Sets the Execution Trajectory

Study start-up — the period from protocol finalisation to first patient in — is the most deterministic phase in predicting clinical trial execution quality. Sites that are activated with complete essential documents, trained investigators, and a functioning pharmacovigilance reporting chain perform better throughout the study than sites where start-up was rushed. The quality of the monitoring plan developed during start-up determines whether monitoring activities will be proportionate to site risk or will apply the same monitoring intensity regardless of site performance.

Our clinical operations services cover the full start-up workflow: site feasibility and selection support; site contract and budget negotiation support; essential document collection and TMF filing; IRB/ethics committee submission support; site initiation visit planning and execution; and investigator training package preparation. We apply AI augmentation to the documentation-intensive components — generating site-specific monitoring plan sections, essential document checklists tailored to site country requirements, and site initiation training materials from the protocol — so that clinical operations specialists spend their time on site relationship management rather than document production.

In a source-traceable clinical document, each statement links to the listing row, dataset value or protocol clause it came from, so a reviewer checks the source instead of trusting the sentence.

Monitoring Visit Execution and Reporting

Clinical monitoring visits — whether on-site or remote — generate visit reports that document findings, protocol deviations, action items, and follow-up status. These reports are both operational documents (tracking what needs to be done at the site) and regulatory documents (providing evidence that monitoring occurred in accordance with the monitoring plan). A monitoring visit report that is delayed, incomplete, or inconsistent with the monitoring plan finding documentation is an ICH E6(R3) GCP finding.

Our monitoring visit reporting SLA specifies first draft within five business days of visit completion for on-site visits, within three business days for remote visits. The report template is structured to produce ICH E6 compliant monitoring documentation including visit summary, findings by category (source data verification, regulatory documentation, drug accountability, IRB/ethics compliance, protocol adherence), outstanding action items with due dates, and follow-up status for items from prior visits. TraceDraft assists in generating structured report first drafts from visit notes, reducing the time from visit completion to report submission.

TMF Quality and Completeness Management

The Trial Master File is the documentary foundation of GCP compliance for a clinical trial. At any point during the study, the TMF must be complete, current, and organised — not because an audit is expected, but because ICH E6(R3) requires it. TMF quality drift is endemic in long-duration studies: document filing falls behind, metadata is applied inconsistently, and the TMF completeness metric at year-three of a five-year study may represent only 70% of the documents that should exist, with the remainder in site files, CRO operational folders, or individual monitors' desktops.

Our TMF quality management service maintains the eTMF against defined completeness and quality KPIs throughout the study lifecycle. We conduct quarterly TMF health checks — sampling 50–100 documents against the completeness checklist and metadata quality criteria — and report findings with specific document references and filing actions. For studies approaching study close-out, we provide a TMF readiness assessment that identifies all completeness gaps before the final audit window, so that close-out filing can be completed without the time pressure and quality risk that accompanies discovery of filing gaps during the close-out inspection.
