Every statement in the draft links to its source, such as AE listing 16.2.7, so the reviewer checks it in place.

In short

A Clinical Study Report presents a trial's design, conduct, results and interpretation in the ICH E3 structure. The data-restatement sections — disposition, demographics, exposure, efficacy and safety results — are careful transcription from tables that already exist. Interpretation and benefit-risk reasoning need the medical writer and should not be drafted.

ICH E3 and the CSR Quality Standard

ICH E3 specifies the structure and content for Clinical Study Reports submitted in regulatory dossiers. The guideline defines 16 sections covering study synopsis, study objectives, investigator and study information, study patients, study treatments, efficacy evaluations, safety evaluations, discussion and overall conclusions, and appendices including the patient data listings, AE narratives, and statistical analysis plan. Each section has specific content requirements, and regulatory reviewers assess both the completeness of required content and the quality of the clinical interpretation presented.

The most common CSR quality deficiencies are not in the scientific analysis — they are in the presentation and traceability layer. Efficacy summary tables that do not match the values in the corresponding TLGs. Safety section text that describes an adverse event incidence rate inconsistent with the adjacent safety table. AE narratives in Appendix 16.2 that reference clinical events described differently from the same events in the safety section. These inconsistencies are not caused by data errors — they are caused by a writing process where sections are authored independently and traceability is managed manually.

Check the source, not the sentence
In a source-traceable clinical document, each statement links to the listing row, dataset value or protocol clause it came from, so a reviewer checks the source instead of trusting the sentence.

The AE Narrative Appendix: Where CSR Timelines Break Down

The individual AE narratives for serious adverse events, required in CSR Appendix 16.2, are consistently the rate-limiting step in CSR delivery. For a Phase 3 oncology study with 100 or more SAEs, the narrative appendix represents a substantial authoring task: each narrative must be complete, accurately sourced from the AE listing and concomitant medication data, MedDRA-coded consistently with the listings, and reviewed by a medically qualified reviewer before the CSR is finalised. Narrative authoring begins after data lock, competes with the main CSR body for writer capacity, and frequently extends the CSR timeline.

TraceDraft's safety narrative module generates structured first-draft narratives directly from the SAS AE listings and concomitant medication datasets, using a configurable template that matches the sponsor's narrative format. Each narrative section — Patient Description, Study Drug, Clinical Course, AE Description, Causality Assessment, Outcome — is populated from the corresponding source data fields, with explicit source citations embedded. The narrative author reviews and finalises a pre-populated document rather than authoring from a blank template. The gain scales with volume: a study with a hundred serious adverse events is where it shows, because the work is near-identical each time and the reviewer is checking rather than composing.

Integrated CSR Review Workflow

The CSR review process involves multiple disciplines: the medical writer, clinical operations, biostatistics, regulatory affairs, medical, and safety functions all review different sections of the CSR against their domain responsibilities. Managing this review process through tracked-changes in Word, with version control maintained by the medical writing team, is a consistent source of version confusion, incomplete review coverage, and revision cycle inefficiency.

TraceDraft's integrated review workflow provides a structured review process where each section of the CSR is assigned to the appropriate reviewer, reviewer comments are captured in a structured comment system rather than in document markup, and the author's responses to comments are tracked against the comment record. The review cycle ends when all comments have been resolved or dispositioned — not when the medical writer believes all comments have been addressed. The version history captures every revision cycle, with the final CSR providing a complete audit trail from first draft through to approval.
